An inquiry committee on the escape of 225 prisoners from Malir Jail in Karachi held jail authorities responsible for the incident.
According to the Inquiry Committee report, in the incident, negligence, mismanagement, and poor security arrangements were revealed.
The report said the situation worsened due to the lack of preparation and implementation of SOPs after the earthquake.
The committee recommended action against Superintendent Arshad Hussain Shah and other officers.
The report also held the Inspector General and Deputy IG Prisons responsible for the incident.
The committee further recommended that modern CCTV systems should be installed in jails.
Internal and external doors and walls should be strengthened with high-strength material, the report recommended.
In the report, compulsory crisis response training was also proposed for jail staff.
Paper spray, stun guns, and smoke grenades should be included in emergency stocks.
The committee recommended biometric monitoring of equipment and weapons.
Chief Secretary Sindh approved sending the report to the relevant departments.
